Case Manager / Care Advisor Home Care – Bayswater VIC (Full time)

Who we are

As a home care service provider in Melbourne, Accent Home Care is a division of Tabulam and Templer Homes for the Aged (TTHA) who has been supporting older people, and their family members, for over 50 years.

Accent Home Care has a range of services that extend to government-funded home care packages (HCP), Commonwealth Home Support Programme (CHSP), and in home care services.

What the role is

The Home Care Package Case Manager works in partnership with consumers, carers, family members, and informal supporters to facilitate consumer-directed care and support.

This includes home care support services that may be required to meet the client’s goals and unique needs with the aim of helping them remain living in the comfort of their own home.

The role will involve community visits, referrals, coordination, assessments, monitoring, and case management.

The successful applicant will need:

  • Diploma in Community Services or Diploma in Case Management or have qualifications in either Nursing, Physiotherapy, Occupational Therapy or Health Science.
  • Demonstrated experience working with clients and carers to identify client needs, risks, and strengths to design a care plan, associated budget and service agreement.
  • Demonstrated experience in case management and care coordination for older people, and their carers for the purpose of supporting older Australians to remain living safely at home.
  • Ability to work within a budget and meet agreed performance measures.
  • Understanding of and experience working within the various guidelines and standards including the Aged Care Quality Standards.

You will also need:

  • Current VIC driver’s license and safe driving record
  • Use of own vehicle in sound condition and with comprehensive insurance
  • Current Criminal History Record check – not older than 3 years
  • Have current influenza vaccination
  • Current First Aid Certificate

Home Care / Personal Care Worker – Eastern and Southern Melbourne Metro

About the Role

Accent Home Care is a division of Tabulam & Templer Homes for the Aged who have been supporting older adults for nearly 50 years.

We are a multicultural, not-for-profit home care provider and are seeking passionate individuals with a certificate III or IV in Aged Care, Community Care, or Disability to provide community home care services.

You will need a driver’s license and a reliable car to visit consumers and we will reimburse you for the costs of using your car.

Accent Home Care offers a competitive hourly rate, as well as weekend penalty rates. As a not-for-profit organisation you have the opportunity of salary packaging the first $15,900 of your wage as tax-free.

About the role:

  • As an employee of Accent Home Care, you will get to work with people living in their own homes
  • You will assist older people to access the community such as taking them to the shops, library, and social support groups as well as going out for a coffee or a walk in the park
  • You will assist with domestic assistance, personal care, meal preparation, in-home respite care, and other tasks to assist older adults to maintain their independence

  • Previous experience within Community/Aged Care sector (desired)
  • Able to speak more than one language (desirable but not essential)
  • Certificate III in Aged Care, or disability (minimum requirement)

Community Care Advisor Nurse (Full Time)

About the Role:

Provide nursing care and support to Accent Home Care consumers, their carers’ and chosen representatives. The role will enable consumers to stay at home for as long as possible while ensuring all nursing care and services are delivered within the TTHA service delivery framework, and consistent with the consumer-directed care model.

This role will value add to the Accent Home Care service with the expert community and clinical nursing knowledge.

The Successful Applicant Will Need:

  • Education in the Community Sector
  • Registered as a health practitioner, Australia (Registered Nurse)
  • Desirable: tertiary qualification, experience in Case Management in the Aged Care and or Disability Sector, service coordination and ability to work within a budget, and with developing budgets
  • Current First Aid Certificate would be an advantage

You Will Also Need:  

  • Current VIC driver’s licence and safe driving record
  • Use of own vehicle in sound condition and with comprehensive insurance
  • Current Criminal History Record check – not older than 3 years
  • Have current influenza vaccination
  • Understanding of and experience working within the various guidelines and standards including the Aged Care Quality Standards.
  • Current registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a registered nurse
  • Demonstrated knowledge of Community Aged Care Services and Home Care Packages (HCP)

Community Care Service Coordinator

About the Role:

The Community Care Service Coordinator is responsible for coordinating consumer care using a team of In-Home Care staff and brokerage arrangements for Accent Home Care (AHC).

The role involves liaising with Care Advisers and Home Care Package (HCP) consumers to ensure services are provided in accordance with the consumer’s ongoing assessment needs and In-Home Care staff availability.

Duties will include building relationships with Care Advisers, Consumers, Carers, contracted service providers, and the wider service system, In-Home Care staff, and wider Tabulam and Templar Homes for the Aged (TTHA) team.

Monitoring and reviewing service delivery, scheduling consumer services while using best-matched care staff, and in line with consumer choice. The overall care responsibilities are to provide quality operational home care services while maintaining efficiency and cost-effectiveness and meeting compliance and procedural requirements.

The Successful Applicant Will Need:

  • Education in the Community Sector
  • Desirable: tertiary qualification, experience in Case Management in the Aged Care and or Disability Sector, service coordination and ability to work within budget Current First Aid Certificate would be beneficial.
