Home Care Package Management

Package management is a service that supports the delivery of your Home Care Package, and can cover administrative activities of the package, such as:

  • establishing and managing home care budgets,
  • coordinating and scheduling services and workers,
  • preparing invoices and monthly statements,
  • complying with regulatory and assurance activities.

Care Management

Care management is a service that is provided to you by a Care Advisor. They have a significant role in your Home Care Package which consists of:

  • regularly assess your needs, goals and preferences,
  • reviewing your home care agreement and care plan with you,
  • ensuring your care and services align with other supports,
  • partnering with you and your family or carers about your care,
  • ensuring your care and services are culturally safe,
  • identifying and addressing risks to your safety, health and wellbeing.
